摘要

從參與第27屆健康促進醫院國際研討會，主題為「健康照護高科技與高接觸之平衡：數位化與對話之挑戰與機會」的國際角度，將細觀臺北市立聯合醫院之現況與未來於健康促進醫院之發展趨勢。該研討會圍繞著以高科技提昇健康照護，同時具有溫度的醫療照護，與北市聯醫發展的方向契合；故將探討科技為聯醫所帶來的醫療服務改變，如何著重於全人照護、持續性及跨專業團隊的合作。尤其是醫院與社區資源的連結、健康識能知識轉化創建為溝通平台並架構系統、落實以人為中心、家庭為單位、持續、出院後續追蹤管理、跨領域合作等完善服務，進而影響政策推動，建立制度及規則等，以增加國際能見度及專業知識及經驗交流。

We scrutinize the current situation of the Taipei City Hospital from the global view of the theme of the 27th International Conference on Health Promoting Hospitals and Health Services - The Balance between High Tech and High Touch in Health Care: Challenges and Chances of Digitalization and Dialogue. The conference circulated on the how high tech may improve healthcare and care with temperature. The conference topic this year conforms particularly well with the developmental direction of the Taipei City Hospital. Thus, we explore how scientific technology may change the health services of the Taipei City Hospital and enhance collaboration that focused holistic, continuous, and cross-discipline professional care, especially how the link between hospital and community resources and health literacy can be transformed into to a communicative platform, so that a complete service centered on person and family which provides continuous follow up management after discharge and further affects the promotion of policy, establishment of systems and rules to increase international visibility and exchange of professional knowledge and experiences.
