醫院所衛生教育資料之分析評估

Analysis of Health Education Information in Hospitals

摘要


目的:本研究整理分析衛生署國民健康局委託醫療院所於門診收集之民眾健康相關問題與回覆資料,將門診民眾提出的健康問題加以分類,以作為醫療院所提供民眾衛教時之參考。 方法:衛生署國民健康局九十二年度『醫療院所衛生教育計畫』中,參與計畫的31家醫療院所,每月由門診民眾詢問之健康相關問題選出五題撰寫答案後呈送健康局,共計收集1,848篇文稿。經專家會議討論以器官系統作為健康問題的分類。邀集40位家庭醫學科醫師,經過初審、複審程序將資料加以分類修改。初審結果分為三部分,分別為「可」、「需修改」、「刪除」,再將資料分二階段審查處理,進一步做資料整理、分類與統計分析。 結果:所有民眾詢問之健康相關問題經整理後共計完成1,078篇,佔58.3%。「刪除」的前三種原因以:題意不清或易混淆(33.5%)、格式不符(17.4%)、內容陳述過於簡略(15.4%)為最多。「需修改」的前三種原因:問題的回答敘述不夠清楚(26.8%)、問題可以合併(25.4%)、以及內容陳述過於簡略(14.4%)為最多。經器官系統類型的分類後,以女性生殖系統(20.4%)、內分泌及新陳代謝系統(9.7%)、腦神經系統(9.7%)問題為最多。 結論:醫療院所在提供民眾的衛教資訊時仍需要注意其正確性與完整性。本計畫所建立的民眾詢問健康相關問題資料庫可協助家庭醫師進行門診衛教,並可在網站上供民眾點選及查詢。

The purpose of this study is to analyze the patients’ health-related questions and their replies from medical professionals collected in various healthcare clinics, a health educational program entrusted by Bureau of Health Promotion, Department of Health, R.O.C. (Taiwan) in 2003. The authors examined and classified the questions and answers drawing from this program with a focus to provide more healthcare educational material for future uses. There were total thirty-one hospitals joining this educational program. In the outpatient clinics of these hospitals, healthcare providers were assigned to collect five patients' health-related questions per month and answer with adequate and detail healthcare information. There were total 1,848 sets of questions and answers collected during the whole period of the program. The information was then reviewed by forty family physicians through two-step assessment. In the first step, each set of Q & A were screened as acceptable, need further revision, or unacceptable. In the second step, these family physicians were asked to revise or implement the answers of health-related questions. Finally, the health-related questions and answers were classified and categorized according their individual anatomical organ system. There were 1,078 sets of Q & A been verified and classified by anatomical organ system which was 58.3% of original 1,848 sets. The main reasons for the deleted questions were confusing questions (33.5%), inadequate writing styles (17.4%), and incomplete answers (15.4%). The main reasons for those questions needed to be modified were unclear answers (26.8%), the questions could be merged into another (25.4%), and the answers were too brief (14.4%). Finally, these health-related questions and answers were further categorized into 20 classes by the anatomical systems. Female genital organ system (20.4%), endocrine and metabolic system (9.7%), and nerve system (9.7%) were the most popular systems asked by patients. In conclusion, this study suggests that the accuracy and comprehensiveness should be emphasized when hospitals provide patients with good quality health information. Family physicians can use this health education database to educate their patients in their daily practice. The whole set of information has been put on the internet for further reference.
