兒童新型冠狀病毒感染之中西醫結合治療觀點

摘要

新型冠狀病毒感染造成的嚴重特殊傳染性肺炎對人類的健康造成重大影響。相較於成人，感染新冠肺炎的兒童病患大多症狀輕微，但也有零星重症病患。過去已有一些成人新冠肺炎的中醫研究，然而兒童的新冠肺炎的中醫相關觀點與研究則較闕如。兒童不是成人的縮影，其健康照護有其特別的考量之處，特別是新興傳染病如新型冠狀病毒感染，如果能夠參考中醫過去與瘟疫感染症對抗的經驗，中西醫彼此截長補短，將有助於兒童的健康照護。本文由回顧兒童新冠肺炎之流行病學、病程與症狀變化為始，藉由西醫病理及免疫變化，對兒童新冠肺炎之臨床表現做分析，並根據疾病症狀表現之重點，以中醫六經傳變及衛氣營血辨證進行分期，提出中西醫結合觀點，並擬定建議處方，希冀能夠對兒童新冠肺炎的中西醫結合臨床照護有所助益。

The new coronavirus infection ＂coronavirus disease-2019＂ (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has a significant impact on human health. Compared with adults, the majority of children who have tested positive for COVID-19 have mild symptoms. However, there are also rare cases where children develop severe symptoms. There have been some traditional Chinese medicine (TCM) studies for COVID-19 in adults, but TCM-related viewpoints and studies on children with COVID-19 are relatively scarce. Unlike adults, children's health care has its own special considerations, especially when it comes to emerging infectious diseases such as new coronavirus infection. If we can learn from the past and refer to the experiences of TCM in fighting against plague infections, the combination of TCM and Western medicine can further strengthen children's health care. This article begins with a review of the epidemiology, course of disease, changes in COVID-19 symptoms in children, and analyzes the clinical manifestations of children's COVID-19 through the pathological and immune changes of Western medicine. It also further focused on conducting a staging study of the progression of the Six Channels and syndrome differentiation of Wei, Qi, Ying, and Blood. The goal is to propose prescriptions, based on the consolidated view of TCM and Western medicine, with the hope to be beneficial to the clinical care of children with COVID-19 by integrating traditional Chinese and Western medicine.
