我們報告一稀少病例為手外張受傷情況下,造成遠端橈尺骨關節脫位,骨間膜撕裂合併橈骨頭骨折脫位。遠端橈尺骨關節疼痛合併橈骨頭或頸部移位性骨折應該警示醫師這受傷病例為此病例報告之可能。合併開放性復位併僥骨頭內固定及內固定遠端橈尺骨關節可以使骨間膜有機會癒合,並且使前臂穩定。術後二個月,疼痛緩解後,就開始活動範圍,六個月之後就達功能性程度。 |
English Abstract |
We present a rare case of a patient with a distal radioulnar joint (DRUJ) dislocation, tear of the interosseous membrane (IOM) and a fracture-dislocation of the radial head after trauma of the extended hand. Pain in the distal radioulnar joint with a displaced fracture of the radial head or neck should alert the doctor to the possibility of this injury combination. The combined open reduction/internal fixation of the radial head with the closed reduction/internal fixation of the DRUJ allowed the IOM to have a chance to heal and gave stability to the forearm. Two months postoperatively, range of motion exercises were started once there was pain relief and there was a return to a good functional level after six months. |
