舌根部之成年型橫紋肌瘤

摘要

成年型橫紋肌瘤是源自於骨骼肌之良性腫瘤，臨床上相當少見。一般多出現於頭頸部，其中發生於舌根部之成年型橫紋肌瘤更是罕見。本報告為一名70歲男性，2年多前於其他醫院意外發現舌根部腫瘤，但因症狀不明顯，病人拒絕任何治療。近一年以來，因漸進性之喉嚨異物感及吞嚥困難而至本院求診，經纖維內視鏡檢查發現表面平滑之舌根部巨大腫瘤；頭頸部電腦斷層掃描顯示均質性之巨大腫瘤佔據口咽及舌根之大部分；病理組織切片及隨後手術之標本均診斷為成年型橫紋肌瘤。成年型橫紋肌瘤之診斷，必需依據臨床特性和病理組織檢查；治療以手術切除為主，若能切除完全，預後良好，且甚少復發。

Adult rhabdomyoma is an uncommon benign neoplasm of striated muscle. Although adult rhabdomyoma has a preference for occurring at the head and neck region, occurrence in the tongue base is rare. We presented one such unusual case of 70-year-old male patient who had had lump sensation of throat and progressive dysphagia for 2 years. Flexible endoscopy revealed a huge mass with smooth surface and wide base occupied the tongue base. Computerized tomography showed a homogenous soft tissue mass over the tongue base and protruding to pharyngeal space. Incision biopsy and subsequent transsoral debulking of the tumor revealed adult rhabdomyoma. The patient recovered smoothly with small residual tumor 4 months after surgery. Although rhabdomyoma is a rare soft tissue tumor, it should be one of list in the differential diagnosis of tumors in the head and neck region. Diagnosis of adult rhabdomyoma is based on clinical history and histopathological survey. The differential diagnosis includes granular cell tumor and rhabdomyosarcoma. After complete excision, the recurrence is rare.
