運用症狀處理模式照護一位肺炎合併肌肉無力患者之護理經驗

A Nursing Experience of a Patient with Pneumonia and Muscle Weakness: An Application of the Symptom Management Model

摘要

本文描述一位嚴重肺炎引發呼吸衰竭及敗血性休克患者，脫離危險期後因肌肉無力面臨身體活動功能受損與心理衝擊。照護期間於西元2017年4月21日至2017年5月16日，運用症狀處理模式收集資料進行整體評估，同時為個案建立復健計畫並鼓勵家屬參與復健過程，改善個案肌肉無力症狀、提升身體活動功能、進而增進自我照顧能力與減輕心理困擾。期望藉此護理經驗分享臨床護理師除了重視個案生理問題外，也須注意因生理問題所引發的心理困擾，以提供此類患者整體性照護。

This article describes the nursing experience of a case of severe pneumonia caused by respiratory failure and septic shock due to muscle weakness leading to psychosocial impairment of physical activity and psychological impact when the patient was out-of-danger afterwards. Thus, the author assessed the patient by methods of Symptom Management Model through overall assessments during the nursing period (4/21/2017-5/16/2017). Meanwhile, a rehabilitation program for the patient was established and his family was encouraged to participate in this process to improve the symptoms of muscle weakness, physical mobility, self-care ability, and reduce psychological distress. Moreover, this experience may serve as a reference to nursing staff not only by focusing on physiological problems but also by paying attention to psychological problems caused by psychological distress. Therefore, the author would like to share this case report to provide nursing professionals with a source of reference for holistic care.
