一位學齡前期急性支氣管炎病童之護理經驗

Nursing Experience of a Preschooler with Acute Bronchitis

摘要


本文旨在描述一位4歲2個月大的病童,因罹患急性氣管炎而住院之照護經驗。筆者自97年1月25日至1月29日護理期間,以Gordon十一項健康評估架構為依據,經由身體評估、非結構性的會談、觀察、治療性遊戲等活動,收集相關資料,進而確立護理問題為體溫過高、呼吸道清除功能失效、恐懼。護理過程中,針對病童在陌生環境及未知情境感到的哭鬧及依賴行為,以角色扮演、畫圖等方式,除了與病童建立護病關係外,更提供案母相關的疾病知識與照護技巧,如發燒及呼吸道的照護及拍痰技巧,付諸同感心關懷病童及案母,以預防案母的擔憂、降低病童的住院壓力、增加對護理人員的信任感。提供病童最佳的人性化照護。

This paper aimed at the nursing experience of caring for a 4-year-2-month-old subject admitted attributed to acute bronchitis since January 25 to 29, 2008. The collected data were based on Gordon's 11 functional health pattern assessment via the physical appraisal、non-structural interviews、observation、therapeutic play, etc. Thus, the nursing problems were proved: hyperthermia、ineffective airway clearance、fear. During hospitalization, the frequency of child's crying and dependency in a strange environment and several unknown situations increased, so using pediatric nurse intervents which included the role playing, drawing, and therapeutic play to establish the nurse-patient relationship. Furthermore, the disease knowledge and disease care skills such as fever and respiratory care, and percussion were supplied to the mother. Besides, provide the empathy to the patient and his mom, such as preventing the delayed worry, reducing the hospitalization stress, increasing the trust in the nursing staff, and provide the patient the best humanity care.
