The issue of patient safety has been noticed by people. And then acupuncture is the most invasive treatment that used by Chinese medicine doctor. Therefore, it is important to provide a safer environment. The missing needle during the acupuncture treatment not only makes the patient bear the risk of medical injury, but also becomes a major treat to maintain the quality of acupuncture care and patient safety. This project takes the patient safety as the center of care concept, and analyzes the reasons for missing needles through the current situation. We found two reason. First, lack of common-check mechanism by the doctor and nurse. Second, the numbers of the needle are unconfirmed by the nurse so that it is inconsistent. After revising the standard operating procedures for acupuncture, including increasing the white board for recording the number of needles, and designing the double-checking procedure, the incidence of missing needles was reduced from 0.18% to 0.01%. It is expected that the experience through this project will be effective and sustainable to prevent the occurrence of missing needles and to ensure the safety of patients. We also hope this improvement can achieve the purpose of acupuncture to cure disease or improve the condition. Finally, hope this project can also be a reference for peer to improve.
