摘要

本篇個案報告是探討一位罹患川崎氏症的病童及其主要照顧者之護理過程。護理期間為2008年10月1日至2008年10月11日。運用整體性評估觀察、會談等方式確立個案的健康問題包括：體溫過高、腹瀉、潛在危險性皮膚完整性受損、口腔黏膜改變及照顧者角色緊張等問題。經由密切的監測體溫變化，增加體液補充，口腔黏膜的護理，提供持續性及個別性的護理措施，使個案於住院期間身體及心理達到舒適，與案母建立在互信的基礎上，給予疾病資訊、陪伴、傾聽，讓案母抒發其情緒感受，進而獲得對川崎氏症正確的認知，減輕其焦慮。希望藉由此護理經驗提供護理同仁，照顧此類病童時，提供以家庭為中心的護理，解決病童生理症狀，同時，面對家屬心理層面的問題更是不容忽視。

From October 1 to 11, 2008, the author cared for a child with Kawasaki disease child; his primary family caregiver was his mother. The whole assessment model was used to collect subjective and objective information through observation and interview. The resulting health care problem list included fever, diarrhea, loss of skin integrity, risk of oral mucous membrane lesions, and caregiver stress, especially as the mother did not understand the disease. Nursing measures including monitoring the body temperature, ensuring adequate hydration, and skin care. The aim was to provide continuous, individual care to increase the child's comfort during hospitalization. In caring for the patient, the author simultaneously helped the mother, providing information about Kawasaki disease and listening as the mother expressed her feelings and concerns. In addition to managing the biophysical aspects of Kawasaki disease, nursing care for a child with the disorder requiresattention to the caregiver's psychological needs.
