摘要


臨床上,造釉細胞瘤的增長速度非常緩慢,通常生長期間沒有明顯症狀,一旦發現時大多已形成較大之腫塊。造釉細胞瘤的治療是個複雜的議題,雖然屬於良性腫瘤,但復發率及侵犯性極高;治療方式通常以手術切除為主。然而,病灶經大範圍切除後,重建的部分更顯重要;將腫瘤徹底切除後,並把缺損的區域重建回原有的外觀,盡量保持病患原有的功能及生活品質。本病例報告是一位49歲男性,下顎骨經節段性切除,並使用顱顏骨固定系統之鈦金屬骨板進行重建,再利用病患的前庭軟組織縫合傷口。本病例使用生物相容性高的鈦金屬骨板,此骨板經由電腦斷層掃描並製作出來,使其更能貼合截斷後顎骨表面,恢復面部的外觀。

Clinically, ameloblastoma is growing slowly and usually with no symptoms. Once it is discovered, it is already very large. The treatment of ameloblastoma is a complex issue. Although it is a benign tumor, the recurrence rate is very high. For this reason, the treatment is usually surgical. Therefore, after extensive resection of the lesion, the reconstructed part is even more important, not only to remove the tumor, but also to reconstruct the defect area back to the patient's original appearance, helping him to restore and improve the quality of life. The reported case was a segmental resection of a middle-aged man's mandible and immediately reconstructed using a Fixation System titanium plate, and the remaining vestibular tissue was used to suture the wound. Most cases are reconstructed using free flap and autogenous bone grafts, but this case uses biocompatible titanium which is printed by CT image. It can well fit the remaining bone surface and restore the appearance of the original face.
