運用機器寵物治療於一位機構失智長者精神行為症狀之護理經驗

A Nursing Experience of Using a Robot-assisted Therapy for Behavioral and Psychological Symptoms of an Institutionalized Older Adult with Dementia

摘要

本文係運用機器寵物治療照護一位新入住長期照護機構患有重度認知障礙、嚴重記憶喪失及出現精神行為症狀之長者的護理過程。筆者照護長者期間運用周全性老年評估確立照護問題，主要有認知功能障礙、憂鬱症狀及精神行為症狀等問題。也發現長者對寵物的依戀，因此在照護上以滿足其基本生理需求為基礎，並藉由機器寵物治療活動，提供具備心理、靈性及社會等層面之個別性及完整性護理照護。長者參與四週規律團體機器寵物活動後，白天打瞌睡及夜眠中斷的情形減少，其精神行為症狀發生頻率也明顯地降低，認知功能及定向感明顯提升，情緒及社交互動也獲得改善。希望以此經驗提供長期照護機構護理人員做為失智老人照護策略之參考。

This article described a nursing experience for a newly admitted elder with severe cognitive impairment, severe memory loss, and behavioral and psychological symptoms in long-term care facility. Using comprehensive geriatric assessment, the author identified the elder's care problems including cognitive impairment, depressive symptoms, and behavioral and psychological symptoms. The author also found that the elder had attachment to his pet in the past. The nursing care first was to satisfy the elder's basic and physical needs, and then provided an individualized and comprehensive nursing care with consideration of psychological, spiritual, and social aspects through using robot-assisted therapy. After attending group robot-assisted therapy regularly for four weeks, the elder had reduction on his day-time napping, night-time sleep interruption, and occurrence of behavioral and psychological symptoms. The elder also had improvement on his cognitive function, orientation, emotion, and social interaction. The author wishes to provide this care experience as a reference for nursing staff in long-term care setting when they care for elders with dementia.
