探討音樂治療對日間病房慢性精神分裂症病患人際互動之影響

An Exploration of the Impact of Musical Group Therapy on the Interpersonal Interaction Functions of Day Care Patients with Chronic Schizophrenia

摘要

本文主旨於探討精神科日間病房之慢性精神分裂症患者，使用音樂團體治療對人際互動障礙之改變。本研究進行採類實驗法，單一團體進行前後測比較。參與研究者共28位，以每週2次，每次1小時共進行8次之音樂治療為介入措施，期間以音樂分享、音樂團體活動及團體討論等方式進行，團體成效評值採人際互動功能量表，針對參與研究之日間病患於措施前、措施後，以及措施後4週追蹤各評量一次，分數越高表示人際互動障礙越大。資料經描述統計及一般線性重複量數統計分析(General Linear Model)，研究結果呈現8個研究子項，皆有逐漸下降趨勢，其中【和別人在一起，我說感到不舒服】達到顯著下降之改變(F=3.273, p=0.47)。由研究結果可知音樂治療對日間留院精神病患人際互動之改善有明顯成效，建議日間病房將音樂團體治療納入持續性之常規健康照護模式，以改善慢性精神分裂症患者人際互動上之障礙。

The aim of this study was to explore the impact of musical group therapy on the interpersonal interaction functions of chronic schizophrenic patients in day care hospital. A one-group, pre-test and post-test quasi-experimental design was adopted. A total of 28 study subjects participated. Intervention consisted of a musical group therapy program of four sessions. Each session lasted one hour, and two sessions were held each week. The strategies included music sharing, musical activity, and group discussion. The interpersonal function scale was used to measure the interpersonal relationship functions of schizophrenic patients before and after music group intervention. In addition, four weeks after the post-test a repeat measure was performed for follow-up purposes. The higher the scores on the follow-up measure, the higher the barriers to interpersonal relationships. Data were analyzed by descriptive analysis, pair-t test, and general linear model of repeat measurement analysis. The results showed that the patients' interpersonal interaction functions were improved continually. The item, ”feeling uncomfortable when with other people,” showed significant statistical decreases (F=3.273, p= .047). In conclusion, the musical group therapy was useful and valuable for improving interpersonal relationship functions for patients with chronic schizophrenia in day care hospital. The researchers suggest that mental health nurses consider including musical group therapy to improve functions of interpersonal interactions among patients with chronic mental illness in day care hospital.
