Acute Kidney Injury Caused by Bilateral Kidney Infiltration of Diffuse Large B-Cell Lymphoma: A Case Report

We presented a 62-year-old woman with acute kidney injury upon arrival at the emergency department. Non-specific symptoms such as flank pain and gastrointestinal discomforts were reported. The findings of image studies raised the suspicion of a renal infiltrative process. Renal biopsy, bone marrow aspiration, and positron emission tomography (PET) confirmed the diagnosis of diffuse large B-cell lymphoma stage IV, with involvement of kidney and pelvic lymph nodes. After mini-pulse steroid therapy, partial recovery of renal function was noted. Although acute kidney injury associated with hematological malignancies is common, acute kidney injury that is directly caused by infiltration of lymphoma cells is rare. In patients with unexplained acute kidney injury and enlarged kidneys, further work-up for underlying lymphoma should be considered.
