摘要

本文個案爲中風患者，所使用的家庭健康評佔架構爲參考Wright及Leahey(1994)之Calgary Family Assessment Model(CFAM)及Friedman (1986)的Family Assessment Model予以修正，經由家訪進行資料收集，所呈現之內容包括評估架構、家庭及個人基本資料、家庭發展階段與任務、環境，家庭結構、家庭功能、家庭因應、家庭之優點與弱點、個人和家庭之健康需要，最後並針對個人和家庭健康需要提供護理處理。
