協助一位初產婦妊娠中期早期破水予羊水灌注住院安胎之照護經驗

Inpatient Tocolysis Nursing Care Experience of a Primipara in the Second Trimester with Premature Rupture of Membranes and Assisting in the Amnioinfusion

摘要


懷孕生產對婦女而言是辛苦又欣喜的過程,對於高危險妊娠的孕婦,在產科而言更為相當重要,若未做好準備,對於母親及胎兒會有危險,甚至影響生產結果。本篇描述一位初產婦23週早期破水住院安胎之護理經驗,自2010年5月7日至2010年6月22日照護期間,筆者以觀察、傾聽、會談及實際身體評估等方式來收集資料,並運用Gordon十一項功能性健康型態評估,發現個案安胎期間的護理問題有:潛在性危險性感染、排便型態改變、舒適型態改變、焦慮,筆者運用傾聽、同理心及提供整體性照護且希望透過個別性護理措施,可以讓個案建立信心,減低心中焦慮不安情緒,協助個案渡過整個住院過程,也將此護理經驗提出做分享,盼對往後照護安胎個案有所助益。

Pregnancy labor is a physical demanding yet joyful process for women. In high-risk pregnancy, it is important that the proper preparations have been made to protect both mother and fetus and ensure safe outcome of the pregnancy labor and delivery. This article describes the inpatient tocolysis nursing care experience of a primipara with premature rupture of membranes at 23 completed weeks of gestation During the nursing care period from May 7th to June 22nd, 2010, the author used research methods such as: observation, listening, in-hospital interview, and actual physical assessment to collect nursing data, together with using Gordon's 11 Functional Health Patterns assessment as a guide to help ascertain nursing care issues during tocolysis, including: potential risk infection, bowel movement pattern change, comfortable pattern change, and anxiety. The author used listening and empathy and provided holistic care and hoped through individual measures of nursing care, to allow confidence to build with the patient and mitigate anxiety to help the patient through the process of tocolytic hospitalization. This nursing care experience was proposed and may be discussed and shared, in the hope that it could be beneficial for future care in tocolysis cases.
