運用「臨終照護」概念提升加護病房末期重症病人之照護品質

摘要

臨終照護(end-of-lifecare，簡稱EoLC)，曾被認為是一門藝術，但現在也是一門科學，更是善終的一部分，加護病房是一個常常需要面對死亡衝擊的地方，研究顯示80%的人是在醫院中死亡，2011年三讀通過安寧緩和醫療條例修正案，讓臨終病人可選擇免除醫療維生儀器侵入性的痛苦，讓臨終者可以得到善終，但自許多高科技醫療儀器的發明，延長人的壽命及存活率，但也相對大大降低臨終品質，本專案運用「EoLC」相關概念，藉由完善的臨終照護相關教育訓練，增加相關設備，並建立加護病房臨終照護關懷監測機制，啟動「加護病房臨終照護關懷」醫療團隊，執行高品質臨終照護政策宣導，提升加護病房末期重症病人優質的臨終照護品質，達到病人及家屬生死無憾的目標。

EoLC (end-of-life care) is not only an art but also a science. EoLC is part of Hospice, especially in Intensive Care Unit (ICU) where deaths happen frequently. According to the study, 80% of people passed away in hospitals. Taiwan government passed the medical policy of Hospice and Palliative Care in 2011, which allows the terminal patients to have their free will to choose not receiving invasive treatments at the end of life (EoL). Owning to the advanced technologies, life expectancy has been extended. However the quality of EoLC dropped sharply. Therefore this study used the concept of Hospice to establish a ＂medical team of ICU-EoLC＂ In order to improve the quality of EoL, some education and equipment associated with palliative care are utilized to promote this policy. Thus the purpose of standing on the patients' dignities can be reached.
