摘要

本文描述一位因意外創傷導致脊髓損傷個案，在加護期面臨複雜的身心問題，筆者運用社會支持系統（包括家庭、護理人員及社會團體）協助個案克服肢體活動障礙及心理壓力調適之護理經驗。照顧期間為2010年2月28日至2010年3月8日，透過會談、觀察及傾聽等技巧收集資料，運用羅氏適應模式分析，確立個案主要健康問題有：r呼吸道清除功能失效」、「身體活動功能障礙」及「焦慮」。在護理過程中運用整體護理概念及持續性的關慎，與個案建立良好的護病關係，提供疾病照護相關資訊，並運用社會支持系統協助個案解決問題及壓力調適，減輕焦慮，使個案面對治療，積極正向配合醫護措施及後續復健治療，並於出院後逐步回復正常生活。冀望此經驗能提供臨床護理岡仁對此顯個案照護之瞭解與學習。

This article describes a nursing experience for an ICU patient with spinal cord injury from an accident. The author assisted the patient in overcoming physical disabilities and psychological stress through a social support system, including family, nursing, and social groups. Data were collected through interview, observation, and listening during the nursing period from February 28 to March 8, 2010. Roy's adaptation model was used for data analysis. The patient's major health problems included ineffective respiratory airway clearance, dysfunction of physical activity, and anxiety. To solve these problems, the author established a good nurse-patient relationship, provided disease-associated care information, assisted the patient in solving the problems and coping with stress for anxiety reduction. Therefore, the patient could proactively face medical treatments and procedures, receive rehabilitation, and resume normal daily activities after patient discharge. The author expects that this study report can be shared with clinical nurses who care for patients with the same diagnosis.
