運用倫理四象限評估法解決倫理問題感受差異之初探

AN EXAMINATION OF THE DIFFERENCES IN THE FEELING OF ETHICAL PROBLEMS BY USING THE FOUR QUADRANTS APPROACH OF CLINICAL ETHICAL

摘要


目的:本研究在探討運用倫理四象限評估法於解決倫理問題感受之差異性。方法:本研究為描述性研究以自擬問卷對某醫學中心臨床護理人員進行調查,研究期間自2012 年1 月1 日至12 月31 日,調查結果以SPSS17.0 for window 套裝軟體進行分析。結果:研究發現對倫理四象限適用性同意評分越高者,能解決倫理問題的同意程度也越高(P<.01),其中以運用倫理四象限評估「可更完整了解病人治療計畫及潛在問題」和「醫療團隊對病人照護是正向的」得分最高,而「感覺更能勝任護理工作」的得分最低。另外,不同服務單位及工作時間對於倫理四象限評估法可解決倫理問題感受有其差異性,其中「感覺更能勝任護理工作」,則以內科、加護、安寧病房及急診室同意程度高於其他單位,達顯著統計差異(P<.005)。

Purposes: This study investigates perceptual differences by means of the Four Quadrants Approach of Clinical Ethical Issues to solve ethical issues. Method: This descriptive study used a self-developed questionnaire to investigate clinical nursing at a medical center. The study period was from January 1 to December 31, 2012. This study used the SPSS17.0 software package for Wsindow to analyze the investigation results. Results: This study finds that those who agree more with the adaptability of the Four Quadrants Approach of Clinical Ethical Issues can better solve the ethical issues (P<.01). The scores of the use of Four Quadrants Approach of Clinical Ethical Issues for evaluation: can help to more completely understand the treatment plan and potential problems of patients and medical team's care for patients is positive were the highest, while that of feeling more competent for nursing work was the lowest. Moreover, different service units and work time led to perceptual differences in the use of Four Quadrants Approach of Clinical Ethical Issues as applied to solve ethical issues. The level of agreement with feeling more competent for nursing work of nursing personnel at Department of Internal Medicine, ICU, Hospice Ward, and ER was higher than that of those at other units, and the difference reached significance (P<.005).
