摘要
約束普遍盛行於國內外，雖然使用目的是在保護病人或老人，但背後同時也隱藏著對病人或老人的傷害，因此長期照護開始提倡對於病人或老人的無約束。約束使用已歷經數個世紀，由於眾多因素使得難以推行無約束，因此本文將說明約束使用的情況與原因，和引發的身心負面效應，讓讀者對於約束有正確觀念和知識後，剖析使用的可能因素，在從品質的觀點－結構、過程和結果，三個面向思考改善約束的替代，削弱促成因素，致力推行無約束，接著結合「以人為本」為出發點的創意與科技，協助無約束的推行，希望以品質的出發點，思考更趨完善的方法，能夠徹底地落實無約束和降低推行過程中的窒礙，邁向更優化的長期照護品質。

Restraint is commonly used in long-term care settings in Taiwan and other countries. Although the purpose of its use is to protect residents from harming themselves. Nevertheless, there's potential danger of harm. The history of restraint use can be traced back to many centuries ago. In recent years there has been movements championing for restraint-free care. However, many factors remain great challenges to the realization of a restraint-free care environment. Therefore, this paper will delineate the situations where restraint is used, the underlying causes and the associated negative physical and psychological effects. After laying a foundation accurate knowledge, the author will analyze possible contributing factors of the restrain-use and propose alternatives to provide quality care. With appropriate alternatives and elimination of contributing factors of restrain-use, we can achieve restraint-free care. People-centered innovations and technologies are suggested for the implementation of restraint-free care in long-term care facilities for better of quality of care.
