摘要

患犬為7歲之雄性可卡犬，於就診前一個月畜主發現其身體之鼻吻部出現化膿灶，經地方獸醫院診治後並無改善，轉診本校教學醫院。臨床檢查發現患犬有4-5%脫水及嚴重營養不良，其鼻吻部及右後肢有化膿灶，背側皮膚有多個小結節及皮膚腫塊。經支持性輸液及抗生素治療兩天後患犬病情無改善而死亡。病理學檢查發現其肺臟表面有灰白色多發之斑塊至結節；右心室有心絲蟲成蟲寄生；脾臟及腎臟可見黃白色之凸起團塊。於H&E染色下可見部分肺臟間質、脾臟及腎臟部分實質組織被囊腫樣物質取代，其內有大量淡染之空泡樣物質。經PAS染色下各組織之囊腫樣物質內呈現許多橢圓紅染菌體輪廓。依據菌體型態其及對組織造成之病變確診為犬隱球菌症合併心絲蟲感染。

Cryptococcosis complicated with Dirofilariasis in an outdoor-kept 7-year-old male Cocker Spaniel was reported. Patient was referred to the veterinary medical teaching hospital with an un-healing nasal purulent necrosis noted for over a month. Clinical examinations revealed 4-5% dehydration, malnutrition and an ulcerative necrosis site over 50% of nasal domain. Similar ulcerative lesion was found on the thigh skin. Several cutaneous masses varying in sizes were located on the dorsal skin region. Clinicopathologic abnormalities including leukocytosis, anemia and uremia were detected. Death was found after the failure of antibiotics treatment and parentral nutrition/ fluid supplementations in two days. Necropsy examinations revealed heartworm (n=11) residency in the right ventricle. Lungs showed multiple pulmonary nodules and pale plaques. Firm, tiny nodules lying on spleen and kidney were observed. Microscopically, pulmonary interstitial tissue, renal and spleno parenchyma were histologically replaced by numerous cysts (H&E) within that numerous pink encapsulated oval organisms, some showing budding were evident under further PAS staining. Such tissue invasion was infiltrated with mild neutrophils inflammation. Taken together, canine cryptococcosis complicated with dirofilariasis was diagnosed.
