本文旨在探討一位呼吸器依賴早產兒出院返家的轉銜期照護經驗,護理期間自2016年08月11日至08月22日,筆者以整體性及家庭功能評估為架構,藉由臨床照護、觀察評估及會談,進行資料收集後,確立個案及其家庭之健康問題為:低效性呼吸型態、潛在危險性肺吸入、家庭因應能力增進的準備度。護理期間依早產兒特性,維持呼吸道通暢,促進氧合穩定;餵食過程依個案需求,選擇合宜餵食方式避免肺吸入情形;依家庭因應能力及個別需求,協助父母找出家庭資源,提升照護知能技能,並強化家庭因應能力與信心,讓個案返家得以成功適應居家呼吸器。筆者期待能將此護理經驗,提供護理同仁於臨床照護此類個案及家庭之參考,達到全人及全家的護理目標。
The paper is intended to demonstrate the experiences of transitional care for a premature male infant with ventilator-dependent bronchopulmonary dysplasia. The nursing care period was from 11^(th) to 22^(th) August, 2016. The comprehensive assessment and family function assessment as the mainframe of this report were used. All information was obtained by direct nursing care, physical assessment and evaluation, and family meetings. The major health problems related to the case and his family were as follows: ineffective breathing pattern, risk for aspiration, and readiness for enhanced family coping. During the nursing care period, personalized care interventions for the premature infant were provided. For example: airway maintenance and oxygenation, feeding techniques and preventing chocking and aspiration, supplementation of family knowledge and skills regarding premature infant care, and allocation of particular resources. The above-mentioned aspects were considered to facilitate family coping and successful adaptation to the home care of a ventilator-dependent premature infant. The nursing experience could be shared with other nursing staff when dealing with a similar situation.
