「幻聽症狀之自我處理」於精神分裂症個案之護理經驗

Self-Management Strategies for a Schizophrenic Patient Suffering from Auditory Hallucination

摘要


精神療法(Psychotherapeutics)著重以非藥物的介入方式,協助改善精神分裂症患者的幻聽干擾,其中常用的為行為控制與認知控制。本篇呈現一位發病七年受幻聽干擾的精神分裂症個案,長期重複以自傷行為的方式因應,在二年內頻繁入院五次且無法適應職場工作,護理期間依身體、情緒、智能、社會及靈性五大層面評估,歸納個案於住院期間核心護理問題為:(1)潛在危險性暴力行為朝向自己/不適當的幻聽因應、(2)知覺感受變異/聽覺發生扭曲及改變。筆者於95年4月27日至5月27日一個月的護理過程中,運用精神療法之「行為控制」-教導自我處理技巧達到有效與幻聽共存;「認知控制」-實際應用自我監測方式,察覺負向情緒及思考的存在,矯正自傷行為,最後予轉銜至日間留院以「增加社會化」。出院時個案可持續運用自我處理技巧,增進對幻聽的自我管理能力並未再出現自傷行為,有效提升其生活品質。

The purpose of this paper is to report on the application of self-management strategies to help a schizophrenic patient who suffered from auditory hallucination. During the nursing process, the authors gathered subjective and objective data through conversation and observation. They thus identified two major nursing problems, as follows: (1) The patient's high risk of violence to himself, due to auditory hallucination. (2) Sensory/perceptual alteration (auditory). Using the behavioral control of psychotherapeutics, the authors taught the patient self-help techniques to enable him to live with auditory hallucination, and cognitive control to improve his self-harming behavior. After discharge, he was able to continue to use self-help techniques to reduce his self-harming behavior and raise his quality of life. Psychotherapeutics was observed to be an effective intervention in the care of this patient suffering from auditory hallucination.
