Galeazzi-equivalent fracture in a child - A case report

摘要


Distal radial fracture with distal radio-ulnar joint (DRUJ) dislocation is called Galeazzi fracture. It often happens in adults. However, fracture of distal radial metaphysis and fracture dislocation of distal ulnar growth plate is called Galeazzi equivalent injury. It is rare in children. Since the growth plate is injured, growth disturbance of ulnar bone is a common complication. Here we report a case of Galeazzi equivalent injury in a 14-year-old boy, who presented with pain, swelling and angular deformity in his left wrist after falling down to the ground. Radiographs of the left wrist showed distal radial fracture with volar angulation and distal ulnar epiphysis fracture dislocation to the volar side (Salter Harris type II). Attempt closed reduction failed. Open reduction and internal fixation was performed for both fractures. During open reduction, entrapment of extensor carpi ulnaris was noted. The patient recovered after operation uneventfully. The implants were removed after 3 months when solid union was revealed in follow-up radiographs. After 6 months, the radiographs showed significant negative ulnar variance. The left wrist was asymptomatic with full range of motion.
