Gorham's disease in the thoracolumbar spine-One case report and review of literature

摘要

Gorham-Stout disease (GSD) is a rare osteolytic disease with characteristic of vascular proliferation and bone matrix destruction. The definite pathophysiology of this disease is still unclear. Abnormal bone homeostasis between osteoclast and osteoblast activity was thought to be the key pathogenesis. The spinal involvement of GSD is rarely reported in the literatures. We report one case of GSD with thoracolumbar kyphosis and spinal cord compression. Surgical treatment with vertebral column resection and instrumented fusion were performed for decompression and correction of kyphosis. Progression of the disease and gradual deterioration of neurological deficit were noticed during the 6-year follow-up period. Besides, widely review of literatures was performed. The pathophysiology, medical and surgical treatment options, and the nature course of the disease were discussed. In conclusion, the prognosis of spinal involvement of GSD was favorable initially but variable in the long-term follow-up.
