物質濫用個案併睡眠障礙之護理經驗

本文描述一位28歲女性，因輪班壓力睡眠障礙並有Ketamine濫用情形，因衝動控制差出現自傷情形、睡眠障礙及急迫性尿失禁而入院治療之護理經驗。於2017年9月21日至10月12日的護理期間，運用「精神衛生護理評估」，以觀察及會談方式收集資料，發現個案主要的健康問題為睡眠障礙、急迫性尿失禁及無效性因應能力。在照護期間與個案建立信任護病關係，通過建立治療性的護病關係，教導放鬆技巧、學習因應策略和膀胱訓練，增強其生活適應能力，協助建立正向壓力調適方法及良好睡眠衛生習慣，避免物質濫用期望藉此護理經驗提供護理同仁，照顧睡眠障礙病患時，能評估個案個別性及身心靈需求，提供整體性照護，增加病患生活品質回歸社會。

This paper described the nursing experience of taking care of a twenty-eight year-old women who suffered from sleep disturbance due to shift stress and abuse of Ketamine . She attacked herself and had sleep disturbance and urge urinary incontinence due to poor impulse control. The nursing period was from September 21 to October 12, 2017. In the nursing process, information was collected by means of a psychiatric mental health nursing assessment with an observation and interview methods. Patient's health problems were as follows: (a) sleep disturbance; (b) urge urinary incontinence; (c) ineffective coping. Establish a trustworthy relationship with the patient during the care period., With an establishment of a therapeutic nurse-patient relationship.Teach relaxation techniques, learn coping strategies and bladder training, enhance their ability to adapt to life, help establish positive pressure adjustment methods and good sleep hygiene habits, avoid substance abuse expectations, use this nursing experience to provide nursing colleagues, and take care of patients with sleep disorders. Assess individual cases, physical and mental needs, provide holistic care, and increase the quality of life of patients returning to society.
