摘要


心率是由許多調控系統來控制,而非一成不變的,因此藉由心率變異的分析可提供這些調控機轉與臨床結果的資訊。心率變異的分析法有時域分析法與頻域分析法,心率變異度的分析首先必須把心電圖的類比訊號轉成數位訊號,以便求得精確到5毫秒以上的心跳間期,時域分析法可求得統計指標與幾何指標,頻域分析法則是利用數學的方法,計算心率因為不同的調控系統(因此會有不同的間隔或頻率)所造成心率變異的貢獻度。頻域分析法一般是利用快速傅位葉轉換或自動迴規模型估計法,高頻處的變異是由副交感神經系統調控,低頻處的變異則是由副交感神經系統共同調控,至於極低頻處與超低頻處的變異,其原因尚待釐清。已經有許多報告顯示心率變異度的指標與生理和臨床結果有密切相關性,因此本文特就心率變異度的概念;、測量方法、生理相關性、臨床應用與未來的展望做一個綜論。

Heart rate is controlled by mulitple regulatory systems and is not regular. Thus, the analysis of heart rate variation might offer important information about the regulatory systems and clinical outcome. Methods used in the heart rate variability included time domainand frequency domain analysis. The electrocardiogram recording should be digitized from the analog signals before further analysis since the interbeat intervals should be measured with a precision up to 5 millisecond or less. The time domain analysis can get statistical and geometric measures. The frequency domain analysis evaluates the variation of heaer rate contributed by different frequency under the influences of various regulatory systems. Fast Fourier transformation and autoregressive model estimation, are the most common methods used in the frequency domain analysis. The fluctuation in the highfrequency bands are mediated by parasympathetic control, whereas the fluctuation in the low frequency bands are mediated by both sympathetic and parasympathetic nervous systems. The origin of the fluctuation in the very low and ultra low frequency ranges are less obvious. Physiological and clinical correlation with the various measures of the time and frequency domain have been reported. This article reviews the concepts of heart rate variability, their measurement, physiological correlation, and clinical app ications, and future perpectives.
