幼兒型心室輔助器之設計與動物測試

The Design and Animal Test of Pediatric Ventricular Assist Device

摘要


為改善台大一號心室輔助器小牛實驗中遇到的各項缺失,並製作適合幼兒使用的心室輔助器,故進行此實驗。使用Pro-E 2000i電腦程式仿機翼構造原理新設計葉輪,以改良扇葉間的角度並取得扇葉間的動態平衡,再使用電腦數位控制的自動化機械加工製作新型葉輪,並裝配成心室輔助器進行水工模型體外測試,以確定新型葉輪功率之改善,同時並將原心室輔助器上下腔室死角部位加以修正,以解決血流靜滯、血栓形成的問題,最後以新型葉輪製作小型心室輔助器,並以15-25 kgs的健康成年犬隻進行活體測試,由所收集的心電圖、血壓、血流等生理資訊進行分析,證明其可成功的與心臓血管系統相互匹配,並可於心臓急性衰竭時發揮輔助心室的功能,協助心臓功能恢復正常。由以上研究得知,新型小型心室輔助器具有潛有可應用於幼兒心臓衰竭時成為輔助心室功能的設備。

This study was conducted for the purpose to improve the drawbacks of Tai-Ta NO. 1 ventricular assist device (VAD) met in the calf experiment, and produce a small VAD adapted for pediatrics. After modifying the shape of airplanes wing, the Pro-E 2000i program was used to design a new impeller with improved dynamic balance, and the machine with computer numerical control was used to make the new impeller. After VAD was assembled with new impeller, the improvement of flow rate could be observed under different pressures and speeds with in vitro hydraulic test. The dead space in the upper housing and lower housing were corrected to solve the problem of blood stagnation and thrombosis occurred in the previous calf experiment. Finally the small VAD constructed with improved impeller was used for ex vivo test with canine model. After analyzing the data collected from physiological information of electrocardiogram, blood pressure, and flow, the small VAD was found to be well adapted with the cardiovascular system during canine experiment. The VAD could also support left ventricular function when heart failure occurred. New type of small VAD demonstrate the potential to be used for pediatric circulatory support.
