糖尿病病人長期血糖控制不佳,易引發慢性併發症,在治療上胰島素為有效控制血糖的選擇。2018年調查本單位門診糖尿病病人胰島素注射率為30.4%較2017年33.5%低,呈現負成長,故引發動機成立專案小組。調查病人胰島素注射率低的因素為胰島素的操作很複雜麻煩、擔心胰島素施打很痛、不清楚胰島素的作用及使用胰島素代表病情嚴重。專案執行自2019年7月1至2020年1月31日,目的為提升胰島素注射率至35%。改善方案為修訂胰島素注射轉介流程、運用多元化衛教工具、情境模擬教學及舉辦病友團體。結果病人胰島素注射率由30.4%提升至36.9%,顯示改善策略可有效提升糖尿病病人胰島素注射與對胰島素有正確的瞭解。
The long-term poor blood glucose control of diabetic patients can easily cause chronic complications. Insulin is an effective choice for blood sugar control in treatment. According to the survey in 2018, the insulin injection rate of diabetic outpatients in our unit was 30.4%, which was lower than 33.5% in 2017, showing a negative growth, therefore initiated the establishment of a project team. After investigation, the factors of low insulin injection rate include patients consider the complication of insulin and troublesome to operate; worried about the pain of insulin injection, and unclear about the function of insulin; also they reckon applying insulin meaning serious illness. The project was implemented from July the first in 2019 to January the thirty-first of 2020, aiming to boost the insulin injection rate up to 35%. The improvement plans include revising the insulin injection referral process, using diversified health education tools, situational simulation teaching and organizing patient groups. As a result, the insulin injection rate of patients increased from 30.4% to 36.9%, indicating that the improvement strategy can effectively enhance the insulin injection rate and the rectify understanding of insulin in diabetic patients.
