本文描述一位加拿大觀光客因雙下肢動脈阻塞,即俗稱「腳中風」之護理經過。照護期間為2018年01月25日至2018年02月01日。經觀察、身體評估、直接照護、會談及病歷查閱蒐集資料,運用羅氏適應模式理論收集資料,確立健康護理問題: 急性疼痛、週邊組織灌流改變、焦慮。照護期間教導深呼吸肌肉放鬆、按摩以減輕疼痛及促進組織灌流;主動關懷、接納文化差異及改善語言障礙使焦慮減輕。外籍人口與日增加,醫護人員會面對語言、文化大相徑庭的病人,建議需了解並接納文化差異、提供個別性照護、協助下載手機應用程式幫助個案健康自我管理,冀望藉本文探討護理經驗分享提供做參考。
This article presents the nursing experience in caring for a male Canadian tourist who suffered from lower extremity arterial occlusive disease, as known as leg stroke. The nursing care period was from 2018.01.25 to 2018.02.01. with the application of the Roy's Adaptation Model. Comprehensive information was obtained via observation, physical evaluation, direct nursing, interview, and medical record review. The main health issues identified were acute pain, altered peripheral tissue perfusion, and anxiety. The patient was encouraged to perform muscle relaxation techniques such as massage, to ease pain and improve tissue perfusion. To reduce the patient's anxiety level, solutions were done to overcome cultural and language barriers between the patient and his caregivers. With the increasing number of foreign patients, medical professionals deal with different languages and cultures. It is recommended that nurses should embrace cultural differences, provide individual care, customize multilingual educational handouts, and assist patients to download an application for health self-monitoring. This nursing experience serves as a reference for nurses who encounter foreign patients in the future.
