In this case, a 15-year-old woman with long-term irregular menstrual cycles had acne on her face for over two years. The redness, swelling and purulence of skin lesions had exacerbated, even affecting chest, back, and buttocks, accompanied with dyshidrosis over extremities. This patient had an indicator of polycystic ovary syndrome (PCOS). The ratio of luteinizing hormone to follicle stimulating hormone raised and with other relevant clinical manifestations. The patient preferred using Chinese medicine treatment to improve facial acne and dyshidrosis. The traditional Chinese medicine (TCM) syndrome differentiation for the patient was liver stagnation, spleen deficiency, damp-heat accumulation, kidney deficiency with blood stasis. Thus, the principle of treatment was "coordinating liver and spleen, clearing heat and eliminating dampness, nourishing kidney and promote menstruation." The patient’s skin symptoms had been gradually relieved. This effective case report could provide evidence of the treatment to alleviate worsened acne associated with hormonal imbalance by TCM.
