外因性氣喘病人之血清組織胺與免疫複合體之測定

Detection of Serum Histamine Levels and Immune Complexes in Extrinsic Bronchial Asthma

摘要


從外因性氣喘病人,初診羣12名以及接受減敏治療至少一年以上之27名抽血檢查其血清組織胺與免疫複合體值,並以正常人10名爲對照組。此外又檢查20例成人類風顯性關節炎病人血清中之免疫複合體(其中10名類風濕因子呈陽性,10名呈陰性)。結果如下:(1)正常人之組織胺值爲0.5±0.23ng/ml,免疫複合體值爲14.39±3.97μg/ml;(2)初診羣之組織胺爲0.68±0.46ng/ml,免疫複合體值爲13.33±7.83μg/ml;(3)治療羣之組織胺值爲0.67±0.60ng/ml,免疫複合體值爲14.35±9.16μg/ml;(4)類風濕性關節炎病人但因子呈陽性者之免疫複合體值爲47.91±27.36μg/ml,而因子呈陰性者爲21.67±9.26μg/ml。統計學上,正常人,初診羣及治療羣之組織胺值均無差異。但若以1.0ng/ml爲界限,則發現初診羣有4名(33.3%),治療羣有8名(29.6%)超過此值,而正常人卻無人超過此值。由類風濕性關節炎病人血清之檢查結果顯示,本研究所使用之免疫複合體測定方法甚爲靈敏。統計比較三羣之免疫複合體平均值亦無差異,而超過20μg/ml之人數亦相近。可見減敏治療亦不會增高血中免疫複合體值。又組織胺值與免疫複合體值無相關關係。

Serum levels of histamine and immune complexes were determined in 12 children with newly diagnosed extrinsic bronchial asthma and 27 who have been receiving hyposensitization for more than one year, ten normals adults and 20 adult patients with rheumatoid arthritis (10 were rheumatoid factor-positive and 10 were negative) were included as controls. The serum histamine levels were checked by the method of enzymatic isotopic assay and immune complexes by polyethylene glycol precipitation. The results showed: 1) The serum histamine level for normals was 0.5±0.23ng/ml, 0.68±0.46ng/ml for 12 new asthmatics and 0.67±0.60ng/ml for 27 hyposensitized asthmatics. Although there was no significant difference in the mean serum histamine level among these three groups, only asthmatics (4 out of 12 new cases and 8 out of 27 hyposensitized patients) had serum histamine level higher than 1.0ng/ml, 2) The levels of serum immune complexes for normals was 14.39±3.97μg/ml,13.33±7.83μg/ml for 12 new asthmatics, 14.35±9.16μg/ml for 27 hyposensitized asthmatics, 47.91±27.36μg/ml for ten rheumatoid factor-positive rheumatoid patients and 21.67±9.26μg/ml for ten rheumatoid factor-negative rheumatoid patients. The lavel of immune complexes in patients with rheumatoid factor-positive rheumatoid arthritis was much higher than those in other groups and there was no difference in the level of immune complexes between newly diagnosed asthmatics and hyposensitized asthmatics. 3) No correlation was found between serum histamine and immune complex levels. It is, therefore, concluded that long-term hyposensitization can not suppress the serum histamine level on one hand increase the immune complexes on the other.
