一位青春期白血病病童住院之因應行為

The Coping Behaviors of an Adolescent with Leukemia during Hospitalization

摘要


本個案報告是採田野研究法,在民國86年5月13日至6月11日護理一位青春期急性淋巴性白血病新發病的少年,在其住院期間擔任其主護護士,參與觀察,記錄與病童互動過程,根據十篇行爲過程記錄採內容分析法,探討病童住院期間之因應行爲。研究結果發現病童出現的因應行爲爲:(1)確認行爲:如蒐集行爲、期待行爲、認定行爲;(2)克服行爲:如自我控制行爲、尋求協助行爲、接受處置行爲、抗議行爲、隔離行爲、磋商行爲、代替行爲、和情緒反應;(3)評價行爲:如正向評值和負向評值。護理人員在此除了提供住院化療期間身體的照顧外,還扮演了父母與病童的咨詢者、教育者的角色與功能,陪伴病童一起因應住院的壓力與治療。

This case report was conducted by the field method approach. The writer was the primary nurse taking care of a male adolescent who was first diagnosed with leukemia during his hospitalization. Content analysis was applied to 10 process recordings to identify the boys coping behaviors during his hospitalization. The boy’s coping behaviors were categorized as (1)orienting behavior, e.g. information-seeking, expectation, and identification; (2)coping behavior, e.g. self-control, seeking support, receptivity, protest, isolation, bargaining, substitution, and emotional expression; (3)evaluating behavior, e. g. positive evaluation, and negative evaluation. The findings address the role and function of the nurse as care-giver during hospitalization and as consultant, listener, and educator, to help the child with cancer during his hospitalization.
