Contralateral Neurologic Deficits Following Microvascular Decompression Surgery-A Case Report

顯微腦神經減壓手術後産生對側神經功能缺損之病例報告

摘要


本院做了很多三叉神經痛的顯微腦神經減壓手術,且術後的併發症並不常見。然而,有一個四十六歲的胖婦女爲治療右邊的三叉神經痛在做了腦神經減壓手術後,産生術後的神經功能缺損,其中包含左側顔面神經麻痹,左側耳聾,左側手臂無力和麻木,和步態不穩。在此,我們探討這位病人術後的情況,例如:遲緩甦醒,術後癲癇,氣腦症,及神經功能缺損的可能原因。

Microvascular decompression (MVD) surgery for trigeminal neuralgia (Jannetta operation) is common in our institution, and postoperative complications are uncommon. However; a 46 year-old obese female who underwent MI/U surgery for right trigeminal neuralgia suffered from postoperative neurologic deficits consisting of left facial palsy, left total deafness, weakness and numbness of the left arm, and unsteady gait. Here, we bring forth the patient’s postoperative events, such as delayed emergence, postoperative convulsions, pneumocephalus, and probable causes of the neurologic deficits for discussion.
