牙周病患者血清內免疫球蛋白與補體之研究

Serum Immunoglobulins and Complement Components in Patients with Periodontal Disease

The objective of this study was to evaluate the systemic humoral immunologic characteristics (serum immunoglobulin and complement component levels) in Chinese patients with various forms of periodontal disease. Eighty-four medically healthy subjects, 18—54 years of age, 41 males and 43 females were divided into 4 groups on the basis of gingival index, periodontal index, plaque index and radiographic changes of full mouth periapical films: adult periodontitis (AP, n=25), juvenile periodontitis (JP, n=16), gingivitis (n=18) and control (n=25) groups. Blood was obtained by venipuncture and serum collected by centrifugation after clotting. Serum levels (mg/dl) of immunoglobulins (Ig G, A and M) and complement components (C3, C4 and C3A) were assayed by single radial immunodiffusion method. The data were analysed by the Student’s t-test for unpaired observations. The results were summarized: (1) significant increase in Ig G (p<0.00l) and 1gM (p<0.0l), decrease in C4 (P<0.05) in JP patients, and significant increase in Ig G (p<0.0l) in AP patients as compare to normal control group; (2) also significant increase in Ig G (p< 0.001) and Ig M (p<0.05), decrease in C4 (p<0.05)in JP patients, and significant increase in IgG (p <0.05) in AP patients as compare to gingivitis group; (3) no significant difference in the levels of immunoglobulins and complement components (P> 0.05) between gingivitis and control groups. These findings suggest that JP and AP patients are accompanied with systemic humoral immunologic reaction, with that JP patients exhibit a more intense immune response. However, the humoral immune response in patients with gingivitis is remain within normal limit.
