實時間超音波術測定正常肝外膽管內徑值的再探討

Normal Upper Limit of Extrahepatic Bile Duct Dimension by Real-Time Ultrasonography - A Reevaluation

摘要


由於儀器的發展與技術的改善,對於肝外膽管全態的觀察,已成為以實時間超音波術診斷肝外膽管疾病,所必須努力的方向。但是,肝外膽管處於腸氣圍繞的環境下,以超音波術觀察其全態,並非每位操作者輕易可及的。文獻中大多以7mm做為肝外膽管單一的正常上限值,以判定此管擴張與否;但亦有以8mm為準的。本文收集高雄地區,20歲以上的非肝膽疾病患者或健康者185名,觀察其肝外膽管內徑值,平均為5.3±1.6mm,推算出來之上限值為8.5mm。若依高雄地區人口年齡層分佈情形加以調適,則此上限值為8.1mm。因此,建議8mm為合理的肝外膽管正常上限值。

To evaluate the normal upper limit of the inner dimension of the extrahepatic bile duct (CBDdmax) by real-time ultrasonography, 185 adults (≧ 20 yr-old) without hepatobiliary disease were studied during 1982 (from January to August) by a routine ultrasound check of abdomen. The mean CBDdmax was 5.3±1.6 (SD) mm, and the upper limit was calculated as 8.5 mm. The latter, after adjustment with the population composition of Kaohsiung City (1982) (data provided by the National Health Administration), became 8.1 mm. Since a distance of less than 1 mm is hardly detectable by ultrasound, we suggest that it is reasonable to consider 8 mm the upper limit of a normal extrahepatic bile duct.
