運用表達性藝術治療於一位急性淋巴性白血病青少年無望感之護理經驗

摘要

本文在探討一位15 歲罹患急性淋巴性白血病之個案，面臨化療後經歷身體心像改變及對癌症治療的不確定感，產生莫大的無望感之護理經驗。筆者運用Gordon 十一項功能性健康型態做為評估指引，透過觀察、會談、身體評估及病歷紀錄等方式收集資料，護理期間為2014 年4 月1 日至4 月11 日，確立病人健康問題包括: 自我保護能力缺失、口腔黏膜破損、身體心像紊亂及無望感等。照護期間以症狀護理、使用海綿棒清潔口腔，改善化療後產生的口腔黏膜破損，執行保護性隔離、勤洗手及戴口罩等預防感染發生，個案因化療後副作用造成身體心像改變及對癌症治療效果的不確定感，經常嘆氣、情緒低落而產生無望感，經由轉介藝術治療師，運用表達性藝術治療透過多元的創作，過程中護理師共同陪伴，鼓勵個案藉由作品表達並深入引導其自身的探索及洞察，發現生命經驗不同的面向，調整絕望及負向情緒，改變看待自己的觀點，漸進轉化生命新的意義，進而認同自我，增加其自我價值感。建議護理人員在照顧類似個案時，可透過跨領域合作導入藝術治療，藉由各項藝術作品抒發情緒，幫助個案正向面對治療，促進自我轉變及成長，達到良好的心理調適，以提升生活品質。

This article describes a nursing experience of caring for a 15-year-old patient with acute lymphoblastic leukemia who exhibited body image changes and feelings of hopelessness after chemotherapy. Nursing care was provided from April 1 to 11, 2014. Data were collected through observation, interaction, interviews, physical assessment, and medical records. Gordon's 11 Functional Health Patterns Assessment guide was employed to identify the following four health problems: lack of ability for self-protection, oral mucosal damage, body image disturbance, and feelings of hopelessness. During the nursing process, mouth care and protective precautions were applied to improve oral mucosal damage and prevent infections, respectively. Through the process of expressive arts therapy, the patient explored his life experiences, and adjusted his emotions, and self-awareness. Finally, the patient became more confident. On the basis of the results of this study, we recommend that nursing be combined with expressive arts therapy to care for seemingly hopeless adolescent cases.
