本研究探討護理人員對實施混合照護模式,加入團體照服員協助非專業性照護工作的看法。以南部某醫學中心內、外科病房及急診之護理人員為研究對象,資料取自護理部資料庫。研究結果:一、在41 項照服員執行之工作項目,約有52% 的護理人員認為「最常委派之工作」、同時亦為「能分擔護理人員工作負荷」之工作項目共7 項;二、護理人員對照服員制度之評量:對照服員協助臨床照護之感受及與照服員工作關係之感受兩大面向,有85% 以上呈現正向評價;在滿意度上對於照服員之專業素質及工作表現,分別有81% 及84% 護理人員表示滿意;三、分析護理人員基本資料對評量結果之影響,僅在「科別」有顯著差異,顯示內科較急診及外科護理人員對照服員有較正向的評價。結論:本研究結果顯示護理人員對現階段混合照護模式大多抱持正向評價,肯定照服員可分擔常規性、勞力性非專業之照護工作,這樣的照護模式有助於增進護理人員從事較高層次之專業照護活動。建議照服員必須接受臨床照護常用技術的訓練與稽核,以增進其專業素質,提昇護理照護品質。
Introduction: This purpose of study was to explore the views of nurses regarding the implementation of a skill mix model and the involvement of group caregivers in assisting nonprofessional care. A total of 296 nurses were recruited from the medical and surgical wards and emergency room of a medical center in southern Taiwan. Results: First, among the 41 items, approximately 52% of nurses considered "the most frequently assigned work" and "sharing nurses workloads" had 7 items. Second, nurses' evaluations of the skill mix model were positive and they expressed satisfaction with the performance of health care assistants. No differences were observed in the nurses' age, working year, education, level of seniority, or other parameters except for "section" which differed significantly. We compared the different sections and found that medical wards attained higher satisfaction than did surgical wards and the emergency unit. Conclusion: The results showed that most nurses reported positive evaluations of the skill mix model. The health care assistants were able to perform routine labor-related nonprofessional care, and this care model assisted the nurses by giving them more time for professional care. Furthermore, the health care assistants should receive additional training and auditing of clinical care techniques to enhance their patient care quality.
