提升護理人員實證護理能力之改善專案

Project to Improve Evidence-Based Practice Competence Among Nursing Staff

摘要


以實證為基礎被視為提升醫療照護品質方式之一,但多數臨床護理師常因工作忙碌、不會運用資料庫資源,以及缺乏文獻評讀概念等原因而降低學習動機。為改善本單位護理師對於實證護理應用認知、態度及行為,進而提升其實證護理能力,專案小組成員經由原因分析後介入遊戲化教學改善,包括:推派人員參加院內實證種子培訓、以遊戲方式設置實證5A流程、改變每年讀書會舉辦方式、訂定單位獎勵制度及建立單位實證LINE群組。改善方案介入九個月後,本單位護理實證能力由改善前56.2%提升至74.1%、認知由64.2%提升至92.1%、態度由82.5%提升至92.5%、行為由63%提升至90%。經由此專案改善顯示,運用遊戲化教學提升護理人員實證護理能力是有成效的,建議日後醫院在職教育課程,可運用遊戲化教學方式,增加護理人員學習動機,進而提升臨床照護品質。

Evidence-based healthcare is regarded as an efficient method to improve the quality of healthcare provision. However, many clinical nursing staff are unwilling to learn the skills because of a lack of time, difficulties in using electronic databases, and difficulties in critically appraising the literature. This project aimed to improve the knowledge, attitudes, and behavior toward evidence-based healthcare of the nursing staff in one unit, thus enhancing their evidence-based nursing ability. After analyzing the causes of the aforementioned difficulties, a gamified teaching program was implemented. This program included the following: (1) Promotion of personnel to participate in evidence-based healthcare teacher training courses. (2) Gamification of teaching programs based on the 5A process of evidence-based healthcare. (3) Modification of the annual evidence-based healthcare reading group. (4) Establishment of a reward system for the unit. (5) Creation of LINE groups for sharing evidence-based healthcare information. After 8 months of intervention, the evidence-based nursing abilities of the participating staff improved from 56.2% at baseline to 74.1% after the intervention. Knowledge improved from 64.2% at the baseline to 92.1% after the intervention. Attitudes and behavior toward evidence-based nursing also improved (from 82.5% to 92.5% and from 63% to 90.0%, respectively). These results indicate that the application of gamification to teaching programs can be an efficient method to improve the evidence-based healthcare competence of clinical nursing staff. The future application of gamified teaching programs to continuing education is recommended.
