兒科護理人員核心能力及其影響因素之探討

Explore the Core Competencies of Pediatric Nurses and Its Influencing Factors

摘要


本研究旨在瞭解兒科護理人員專業核心能力現況及相關影響因素,以作為日後建構護理人員核心能力訓練模式之依據。本研究為橫斷式研究設計,採方便取樣,研究對象為110位北部某醫學中心兒科病房到職2年以上護理人員,經其同意後以基礎護理核心能力量表及進階護理核心能力量表進行問卷調查;在受試者完成問卷後,再由單位護理主管以相同問卷評量受試者的基礎與進階護理核心能力。研究結果顯示,護理人員自評「基礎護理核心能力」為154.88分(SD=18.16),「進階護理核心能力」為67.30分(SD=9.96);護理主管評量護理人員「基礎護理核心能力」為162.73分(SD=15.97),「進階護理核心能力」為69.61分(SD=9.83),護理人員自評與主管評核兩者間無顯著差異(p=.291);在「基礎護理核心能力」不論是護理人員自評或護理主管評核皆以資訊技術的利用能力最高,基於實證的專業執行能力最低,但在「進階護理核心能力」方面,護理人員自評以工作執行能力最高(3.20分,SD=0.22),護理主管評核則是自我管理能力最高(3.52分,SD=0.15),而研究能力在護理人員自評及主管評核皆為最低。在相關影響因素分析方面,護理人員教育程度、級階與基礎護理核心能力有顯著相關(r=0.216,0.226; p<.05);年齡、級階、總年資、單位工作年資與進階護理核心能力有顯著相關(r=0.401,0.408,0.294,0.293; p<.001)。建議未來對於到職2年後兒科護理人員,繼續加強護理實證或研究訓練,鼓勵其參加相關訓練班,輔以實務經驗,提升實證與執行研究的護理核心能力。

The purpose of this study was to analyze the professional core competencies of pediatric nurses and identify the factors that influence the development of these competencies. This study adopted a cross-sectional research design and employed convenience sampling to recruit 110 pediatric nurses from a medical center in northern Taiwan. Only nurses who had worked in the pediatric ward for more than 2 years were recruited. The nursing staff self-assessed their basic nursing core competence and advanced nursing core competence, and the average total scores were 154.88 (SD= 18.16) and 67.30 (SD=9.96), respectively. The nursing supervisor also assessed the nursing staff's core competence and advanced nursing core competence, and the average total scores were 162.73 (SD=15.97) and 69.61 (SD=9.83), respectively. The scores of the self-assessments and supervisor assessment were not significantly different (p= .291). In terms of basic nursing core competencies, the self-assessment scores and scores assigned by the nursing supervisor were consistent. However, in terms of advanced nursing core competencies, the nursing staff self-assessed their job execution ability to be the highest (3.20, SD=0.22), whereas the supervisor rated their self-management ability to be the highest (3.52, SD=0.15) and research ability to be the lowest. The study also explored the relationships among several factors and the core nursing competencies of the nurses. Education level and nursing ladder were significantly associated with basic nursing core competency (r=0.216, 0.226; p< .05), whereas age, nursing ladder, total seniority, and unit working experience were significantly associated with advanced nursing core competency (r=0.401, 0.408, 0.294, 0.293, respectively; p< .001). The study findings indicate that senior nurses should be encouraged to practically apply their knowledge or participate in research training courses to enhance their core nursing skills, particularly those related to evidence-based practice and research execution.
