Studies on Motor Nerve Conduction Velocity in Chronic Uremia

慢性尿毒患者神經傳導速度之研究

摘要


為探討國人慢性尿毒患者之周邊神經病變的流行率及致病機轉,吾人研究16位定期血液透析尿毒患者之運動神經傳導速度。以8位患者為一組,分成兩組。第一組平均年齡為26歲,第二組平均年齡則為52歲。所有的患者,皆以右肢後側之脛神經為主,測驗其神經傳導速度。同時檢查其血清肌酸酐、鈣離子、磷酸鹽、鹼性磷酸酶、血中尿素氮及副甲狀腺激素值。有62%的尿毒患者,其神經傳導速度明顯損傷。然而損傷之狀況,卻與血清生化之檢查,皆無有意義之相關性。本研究結論,國人定期血液透析尿毒患者,其臨床之周邊神經病變普遍,而其致病機轉則須進一步的研究。

To investigate the prevalence and pathogenesis of subclinical peripheral neuropathy in Chinese uremics, we studied the motor nerve conduction velocity (MNCV) in 16 uremics on chronic hemodialysis (from 3 to 32 months). They were divided into two groups-group 1 with mean age of 26, and group 2 with mean age of 52 (n=8 in each group). To standardize the results, the values for MNCV in right posterior tibial nerve were used for all patients. Serum creatinine, calcium, Phosphate, alkaline phosphatase, blood urea nitrogen and parathyroid hormone were measured in all the uremics. Significant impairment of MNCV was found in 62% (10/16) of the uremics. However, the coefficient of the correlation (r value) between MNCV and all serum biochemical determinations were all not significant statistically. It is concluded that subclinical peripheral neuropathy is common in our maintenance hemodialysis patients, but further study should be done on its pathogenesis.
