Cementless Total Hip Replacement in Patients with Avascular necrosis of Femoral Head A 2- to 9- Year Follow-Up in Patients Younger than 45 Years Old

Between 1987 and 1993, 78 total hip replacements (THR) were performed on 49 patients, all of whom were younger than 45 years old and all were diagnosed as late stage avascular necrosis (AVN) of the femoral head. The average age at the time of surgery was 37.8 years. The average time of follow-up was 5.8 years ( range, 2.5 - 9.4 years ). Patients were evaluated with modified Harris hip score, as well as serial radiographs. The average hip score at follow-up was 88.5 points, with 82.1% of patients achieving good and excellent results. During the period of follow-up, 5 femoral stems, 6 acetabular cups and 4 polyethylene inserts were revised due to loosening or severe wear. The overall revision rate was 6.4% for components and 15.8% for hips. Thigh pain, though mostly of mild degree, was present in alarmingly 35.8% of patients. Periprosthetic osteolysis was found in 24.4% of hips, mostly of insignificant extent. The result of this mid- to long-term observation on cementless THR in this difficult group of patients was encouraging. However, many challenging problems such as method of fixation, polyethylene wear and osteolysis still remained to be solved.
