斷指再接植手術後之復健治療計劃

近幾年來因顯微手術在技術上的進步及器械的改良，斷指再接植手指的存活率可高達85%以上。但斷指再接植手術的目標不僅要使斷肢存活，功能的恢復更是重要。所以手術後積極接受復健治療，期使手部能發揮最大功能，是相當重要的。我們收集自1990年8月至1993年6月，接受斷指再接植手術的82名病患128指斷指，術後依一定的復健治療計劃，而於街後半年以「Tamai的斷指重接後功能評估表」評估功能恢復情形。在117指存活的再接植斷指中，99指有功能評估結果，其中59指(59.5%)的分數在80至100分之間，36指(36.3%)的分數在60到79分之間，4指(4.2%)的分數在40到59分之間，39分以下者無。然後以Student-t test及one-way ANOVA 來比較復健是否影響再接植斷指的功能，結果顯示：接受復健治療的斷指，在Tamai總分、關節活動度、日常生活活動、病人的滿意度之分數以及總關節活動度均比未接受復健治療者好，且在統計上有顯著之差異。

The purposes of this study were to investigate the effects of rehabilitation for the replanted digits. A well-planned rehabilitation protocol was applied to the patients who received digital replantation in National Cheng-Kung University Hospital, from 1990 August to 1993 June, 128 digits in 82 consecutive patients with replantation were studied. 117 digits were successfully salvaged, and functional evaluation were obtained in 99 of them. All these 99 digits were followed up for 6 months or more, and the functional results were evaluated using Tamai's scoring system. 59 digits were rated excellent, 36 digits were rated good, 4 digits were rated fair, and no digits were rated poor. The functional outcome of rehabilitation group were significantly better than that of non-rehabilitation group (p＜.05). The rehabilitation protocal is effective for the functional recovery of replanted digits.
