The Role of Magnetic Resonance Venography in Cerebral Venous Sinus Occlusive Disease

磁振靜脈造影在診斷腦部靜脈竇阻塞性疾病所扮演之角色的探討

摘要


在本篇研究中,我們分析七個病例之磁振造影與磁振靜脈造影。其中三位病患為不明原因之上矢狀靜脈竇栓塞,磁振靜脈造影於栓塞部位顯現為信號消失,更確定了磁振造影之診斷。另有一例為感染性靜脈栓塞,其磁振靜脈造影除顯現栓塞部位之信號消失外,更偵測出許多的靜脈側支循環。在腫瘤之病人,磁振造影更可將腫瘤與其附近靜脈竇間之立體關係描繪出來。我們的結論為:磁振靜脈造影之診斷可加強與證實磁振造影之診斷,對於懷疑腦部靜脈栓塞之病人,同時施行磁振造影與磁振靜脈造影是必要的。

The magnetic resonance image (MRI) and MR venography (MRV) of seven patients were analyzed in this study. Three of them were with superior sagittal sinus thrombosis. The MRV revealed signal loss in the thrombosed segment and confirmed the diagnosis made by MRI. One patient had thrombophlebitis induced by adjacent subdural empyema. The MRV not only depicted segmental signal loss, but also presented multiple venous collaterals. In cases of tumor, the MRV clearly delineated the spatial relationship between the tumor and the dural sinus. It is concluded that MRV played a role to substantiate the diagnosis of MRI. A combination of MRI and MRV examinations is mandatory in a patient with suspected cerebral venous occlusive disease.
