摘要


本文報告三例硬腦膜靜脈竇栓塞之磁振造影發現。 栓塞之確立診斷乃藉由頭部動脈血管攝影(2例)及頭部電腦斷層掃描(l例)證實。三例之磁振造影皆顯示有高強度訊號於靜脈竇內,因而取代正常因血流產生的低強度訊號。 以磁振造影診斷此疾病,可能發生錯誤的情況有兩種。第一是因血流造成的假影,第二是栓塞血塊的訊號變化。兩者皆可藉由適當的造影程序而區別之。 磁振造影更有多種不同的成像方式如spin echo, gradient echo, phase imaging及MR angiography等,而能獲得較傳統診斷方式更精確之診斷。 因為磁振造影有非侵襲性及可直接觀察到栓塞本身這兩個優點。在疑有硬腦膜靜脈竇栓塞之情況下,磁振造影應為最佳之診斷方式。

The magnetic resonance images (MRI) of three patients with dural sinus thrombosis (DST) were reviewed. The diagnosis was verified by computed tomography (CT) and angiography in two patients and by CT in the third patient. In all patients, high signal intensity was seen within the affected dural sinuses replacing normally seen flow void low signal. The pitfalls, such as MRI flow artefact and blood clot degradation signal change, can be avoided by proper strategy. The diversity of available MRI sequences like spin echo, gradient echo, phase imaging and MR angiography make the diagnosis more accurate. Due to non-invasiveness and direct visualization of the thrombi, MRI should be the modality of choice in cases suspected of dural sinus thrombosis.
