摘要

本文報告一位19歲男性廣泛性食道平滑肌瘤增生的病例。病人有三年慢性進行性吞嚥困難的病史，在後前位胸部Z光片呈現波浪形變寬的肺中隔，側位胸部χ光片顯示在支氣管壁後方有增厚的軟組織陰影並造成支氣管向前彎曲的現象，鋇劑食道攝影檢查發現食道黏膜完整，在上段及中段處有多個平滑邊緣的小葉狀突起病灶，胸部電腦斷層攝影顯示一段相當長且對稱增厚的整圈軟組織腫瘤包圍著食道壁，近端食道腔擴大並有氣體液體介面。到目前為止，這是一個少見且僅有少數病例被發表過，我們討論其臨床表現及影像學特徵，並回顧文獻。

A case of diffuse esophageal leiomyomatosis in a 19-year-old man with a 3 years history of slowly progressive dysphagia is presented. The chest radiograph showed a widened and undulated mediastinal contour. A barium esophagogram demonstrated a multilobulated protuberant lesion with a relatively smooth margin in the upper and middle thoracic esophagus. Contrast-enhanced computed tomography (CT) of the chest revealed a long segment of circumferential soft tissue mass encircling the upper and middle thoracic esophagus with prominent dilatation of the proximal esophageal lumen containing air- fluid levels. Multiple leiomyomas of the esophagus were later proved on esophagectomy.
