功能性內視鏡鼻竇手術的併發症
Complications of Functional Endoscopic Sinus Surgery
摘要
功能性內視鏡鼻竇手術自Messerklinger 1978年發明用來治療慢性鼻竇炎，已是當前最佳方法及主流，雖然這手術所引起併發症比率比傳統鼻竇手術爲低，如果手術時加上導航系統的使用，更可降低併發症的發生率。但併發症仍時有所聞，大部份皆屬輕微的併發症，但也有嚴重的併發症發生，如失明或甚至死亡等。本文就以往文章及報導所統計可能發生的併發症，依手術時及手術後分類，手術時最常發生以術中出血，其次如眼眶周圍及眼眶內積血、脂肪顯露、複視、視神經受損、鼻淚管受傷、腦脊髓液外漏及嗅覺神經受損等，而手術後出血、腦膜炎、腦膿瘍、黏膜黏連、鼻竇開口堵塞或狹窄、息肉復發、萎縮性鼻炎等，分別將發生原因、預防方法及善後處置提出討論。

The functional endoscopic sinus surgery has been the primary surgical advance in the treatment of chronic paranasal sinusitis. Though the complication rate of this procedure is not high, the severe complications could happen during or after the operation. The severe complications include blindness, stroke and death, etc. This article not only discusses the intraoperative and postoperative complications, but also focuses on the avoidance and treatment of the complications resulting from the functional endoscopic sinus surgery.
