鼻竇手術引起之眼部併發症
Ophthalmic Complications Following Paranasal Sinus Surgery
摘要
雖然鼻竇手術的併發症很低，但其造成之眼部傷害，傷有3%之多。這些手術併發症，包括有眼窩水腫、眼窩出血、眼球內陷、外眼肌肉運動障礙與鼻淚管阻塞等等，但是最嚴重之併發症為視力喪失，由於中心視網膜動脈阻塞，或壓迫性視神經病變，或直接傷及視神經引起。鼻竇手術當中，要及時發現對眼部之傷害與適時的內、外科醫療補救，才能使眼部的傷害減為最低。本文報告四例因接受鼻竇引流手術，引起眼部併發症的罕見病例。

Because of the intimate anatominical relationship between the orbit and the paranasal sinuses, orbital and ocular damages may be seen in all of paranasal sinus surgery. These complications include orbital edema, orbital hemorrhage, enophthalmos, motility disturbances, and nasolacrimal duct obstuction. The most serious complication is blindness, usually resulting from optic nerve compression, central retinal artery occlusion, or direct injury to optic nerve itself. Early recognition of such iatrogenic ophthalmic injuries and an understanding of their nature and pathogenesis are important in the management of these complications. Four patients are presented with ophthalmic complications following paranasal sinus surgery for chronic pulurent sinusitis.
