動力濾淨式呼吸防護具於耳鼻喉頭頸外科之臨床應用建議

自2019年新型冠狀病毒（severe acute respiratory syndrome coronavirus 2, SARS-CoV-2）於全球各地廣泛地傳播，台灣社會也於2021年5月發生嚴重特殊傳染性肺炎（coronavirus disease-2019, COVID-19）大流行，進而影響國民健康與民生經濟甚鉅。耳鼻喉頭頸外科醫師，身為上呼吸道的處理專家，時常執行可能傳播新型冠狀病毒的相關醫療處置。保護醫療人員免於染疫，以保存醫療量能是感染控制與醫療政策重要的一環。個人防護設備（personal protective equipment），包含拋棄式手套（disposable gloves）、拋棄式隔離衣或圍裙（gowns or aprons）、面罩和護目鏡（face shields and goggles）、N95口罩（N95 mask）以及動力濾淨式呼吸防護具（powered air-purifying respirator, PAPR）等，皆需因處置特性、暴露風險與執行場所現況，選擇穿戴裝備不同層級個人防護設備。其中，PAPR較少於台灣耳鼻喉科界使用；因此，於本文介紹PAPR的相關文獻與台大醫院的使用策略與流程，以期能作為疫情期間各位耳鼻喉頭頸外科醫療同仁，個人防護裝備的使用參考。

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has widely spread all over the globe since 2019. Taiwan society also started to experience the coronavirus disease 2019 (COVID-19) pandemic in May 2021, with a catastrophic impact on people's health and domestic economics. As the experts in upper airway management, Otolaryngology-Head and Neck surgeons are facing SARS-CoV-2 and the related aerosol generating procedures in the front line. Protecting healthcare personnel from infection is a critical and important point in infection control and healthcare policy. Recently, the related topic has attracted public attention. In particular, powered air-purifying respirator (PAPR) is less familiar within Taiwanese Otolaryngology-Head and Neck practitioners. Herein, we reviewed PAPR-related article from the literature and introduced the PAPR instructions of National Taiwan University Hospital. We hope this article will provide some timely and practical advice and reference for our Otolaryngology-Head and Neck Surgery colleagues.
