喉部發炎性肌纖維母細胞腫瘤造成緊急上呼吸道阻塞－病例報告

發炎性肌纖維母細胞腫瘤是一種罕見的間質細胞良性腫瘤，很少發生在頭頸區域，最常在腹腔與肺部發現。本病例報告記載1名64歲女性因為聲帶瘤造成急性呼吸道阻塞和院內心臟無脈搏性電器活動，經過緊急心臟復甦術而後病患回復自發性血液循環。後續進行機械手臂經口切除聲帶腫瘤，病理切片顯示為發炎性肌纖維母細胞腫瘤。3個月後追蹤內視鏡發現新聲帶腫瘤，病理切片顯示為鱗狀上皮細胞癌，後續安排同步放射線治療與化學治療。發炎性肌纖維母細胞腫瘤可能造成急性呼吸道阻塞，也有惡性轉變的可能，需要特別注意。

Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal benign tumor that primarily affects the head and neck region. Most occurred in the abdominal region and lung. This case report describes a 64-year-old female who developed a vocal cord tumor, resulting in respiratory failure and pulseless electrical activity. After consequent cardiopulmonary resuscitation at the emergency room, the patient returned to spontaneous circulation. Trans-oral robotic surgery resection of the tumor was conducted, and pathology showed IMT. However, after 3 months, the vocal cord tumor recurred and the pathology showed squamous cell carcinoma, and further concurrent chemoradiotherapy was arranged. The prevalence of a benign IMT can obstruct the airway and lead to malignancy, which would require particular attention and treatment.
