眼化學藥品灼傷之臨床經驗

Clinical Observation on Ocular Chemical Burn

並列摘要

The author presented the clinical observation on 22 eyes (15 cases) of the ocular chemical burn, including acid burn 12 eyes (8 cases) and alkali burn 10 eyes. (7 cases). Acid burn cases were divided into two groups according to their cause. The first group was that caused by others with criminal intention, and so the ocular damage was rather serious because of larger amount of acid used and the concurrent involvement of neighboring skin of the face. The second group was that caused accidentally, and the ocular damage was mild. In alkali burns, all cases were caused accidentally by splashing. Acid burns differed from alkali burns in their chemical property and the destructive action on the ocular tissue. Although Burns caused by acids looked severe at the beginning they were never as serious as those caused by alkalis. They were essentially non-progressive and late relapse were uncommon. Their special characteristics were poor penetration into the eyes, little tendency of exudative manifestation and the corneal epithelium to desquamate, marked tendency of the relatively opaque cornea to heal with rather sharp demarcation of the lesion, and good prognosis with less complication. In contrast to acid burns, the alkali burns were progressive in nature and had the tendency to late infiltration and ulceration.
